Searching by Address

When you first start the tool, you see a screen like this:

 

 

To search for health centers by address:

 

·         Click searchbyaddressto display the address information dialog, if it is not already shown.
 

·         Enter the as many parts as possible of the address, city, and State you want to use as the beginning point for your search.  
 

·         For addresses, enter the physical street address, not a Post Office Box number or Rural Route number.
 

·         Do not enter apartment or unit numbers as part of the address; only the street address is needed.
 

·         If you know it, you can also enter the ZIP code to help in the locating process, but it is not required.

 

·         If you search only with ZIP code, the tool will search outward from the ZIP code you enter, until it finds at least one health center or until the maximum search distance is reached.  Thus, you may get results listed for ZIP codes other than the specific one you entered.
 

·         When you are ready, click the Find Health Centers buttonbutton to start the search.
 

 

NOTE: If the information you enter matches more than one possible starting location, the tool will give you a list of all the possibilities.  Choose one of the items in the list to begin the search from there.  For example, the underlying Geocoding process will find a single match for input “Baltimore County, MD” and will search health centers in that county.  While searching with “Baltimore County” without the State information the system will find multiple candidates as illustrated below. Once user picks the intended one, the system will search health centers near that location.
