About Find a Health Center

Find a Health Center helps you search for HRSA-funded health centers. If you have geolocation turned on, you will automatically see Health Centers near you. Or, you can search near a specific place by typing a city, ZIP code, or a specific street address into the Location box; then click the magnifying glass or press enter/return on your keyboard.

The search area is set to a 5 mile radius when you begin, but you can set a wider search area if you like.

You can export results to a printable PDF file. Only the 25 closest sites will appear in the PDF export.

Note To HRSA Health Centers

HRSA Health Center Grantees and Look-Alikes, please note:

1. The Health Center information represented on this website IS NOT representative of a Health Center’s complete Scope of Project.
2. If a Health Center identifies errors in the contact information of a Service Delivery Site, please follow the following instructions to fix the error:

Grantees: Registered users with access to the EHBs Grant Handbook can update the service delivery site information in the EHBs.

1. Log in to the EHBs (https://grants.hrsa.gov/webExternal/login.asp) and click the “View Portfolio” link in the left menu.
2. The list of grants you have added to your portfolio will be displayed. Click the “Open Grant Handbook” link for the Health Center program grant. (Grant number starting with H80)
3. The Grant Handbook overview page will open. Click the “Sites” link in the left menu.
4. The list of Sites in Scope will appear listed. Locate the site for which you want to update the information and click “Update”.
5. The Update Site page will open. Make the necessary changes by selecting the “Update” button in the appropriate section.
6. Once you have made all changes, click “Save and Continue” to save your changes.

Look-Alikes:

1. Log in to the EHBs (https://grants.hrsa.gov/webExternal/login.asp).
2. Click on the FQHC-LALs tab.
3. Click on the “View Portfolio” link in the FQHC-LAL Home drop-down menu.
4. Click on the “Open Designation Handbook” link.
5. Click on the “Update Sites Information” link.
6. The list of sites in scope will appear listed. Locate the site for which you want to update the information and click “Update.”
7. The Form-5B Edit page will open. Make the necessary changes by selecting the “Update” button in the appropriate section.
8. Once you have made all changes, click “Save and Continue” to save your changes.
